Our geriatric care managers and care coordinators are responsible for managing and coordinating various services that may be involved in the client’s home care.
Care Co-ordination Services include:
- Constant communication to keep the client and their family informed
- Scheduling home visits with therapists and nutritionists
- Accompanying client to doctor appointments
- Arrangements for the renewal and pick-up of prescriptions
- Arrangement for the delivery of medical equipment and supplies (i.e. wheelchairs, commodes, walkers, hospital beds and diapers)
- Arrangements for continuous food delivery
- Continuity of care throughout transitions (between home, hospital and assisted living facilities).
